2011-2012 Medical Forms are Now Available!All incoming freshman and transfer student-athletes must complete the entire New & Transfer Medical Information Packet and have a physical examination prior to practice, competition, and participation. The physical examination will be administered by our team physician when his/her team is assigned. Returning student-athletes must complete the entire Returning Athlete Medical Information Packet and attend a physical when his/her team is assigned. Dates and times for each sport's physical are posted below. Our athletic physical examination will be conducted by our team physician to clear the athlete for intercollegiate athletics. A physical exam by a personal physician will not be accepted. Fall Sports Physicals will be on Monday, August 8, 2011. Click here to see the schedule.
